Licensed in Another State

 License by Endorsement is for applicants who have an active license in another state, at the same practice level and in the same discipline, for at least 1 year.

Application fees are non-refundable.

Applicants for Licensure by Endorsement shall submit:

  • A complete set of fingerprints on a blue lined FD-258 card, or verification of a fingerprint clearance card from the Arizona Department of Public Safety ("DPS")
    • To request a blue lined FD-258 card be mailed (include address): [email protected]
    • It may take up to 2 weeks for the card to reach you. As an alternative, you may try going to your local police station to see if they can roll your fingerprints onto the blue lined FD-258 fingerprint card or do an online search for "fingerprint services" to see what is close to you.
  • A completion certificate for the Arizona Statutes/Regulations Tutorial
  • A copy of a drivers license or state issued ID
  • Verification of any professional credentials
  • Masters degree transcript (if applying for LCSW, LPC, LMFT or LIAC)   
    • See Applicable Degree information
    • Official transcripts may be requested to be sent to [email protected]
    • Or an official transcript may be mailed if it is in a sealed, official envelope

Applicants are highly recommended to have the above items available prior to beginning the application process. Fingerprints must be mailed or dropped off to the Board, but most other documents can be scanned and attached to the online application.

Background questions and signature must be completed by the applicant, not on behalf of the applicant. Applicant must provide their email for Board correspondence.
